GDPR and Client Contract

​In order to start counselling, you must read and accept the below.

Client contract

1 ) Counselling lasts 50 minutes per session, unless shorter sessions are requested.

2 ) Entitlement is to  6 sessions of therapy.

3 ) To cancel or reschedule a session, at least 24 hours notice must be given by to the counsellor or administrator.

4 ) A late cancellation or a no show will count as one of the allocated sessions.

5 ) Two late cancellations or no shows, we will have to stop the counselling.

6) If you/ the young person cannot attend the appointment, it must be cancelled no later than 24 hours before the appointment time (please email admin@stopbreathethink.org.uk). Failure to attend a session without notifying us may result in permanent discharge from the service

7 ) If counselling stops because of late cancellations or no shows, re-referring in the future is possible but a drop in call with one of our administrators to discuss commitment to the service and the sessions will be required.

8) In cases of lateness, the session will still end at the agreed time, and a shorter session will be delivered. 

9 ) If either the young person or counsellor feels that the working relationship has broken down, then a referral will be made back to admin@stopbreathethink.org.uk and  a new counsellor will be allocated.

10 ) Sessions will not take place if a client under the influence of either alcohol or drugs.

11 ) A maximum of two weeks absence of regular sessions, due to illness or holiday, will be accepted. However, after this time appointments will not be held open and a re-referral will need to be made to admin@stopbreathethink.org.uk.

12 ) Counsellor and client to agree the most suitable method of contact between sessions.

13 ) If cut off during a session, the counsellor will try to reconnect using the same method of communication. If this is unsuccessful the counsellor will contact  the client on the emergency contact details provided.​

14 ) All information shared during counselling sessions is confidential, meaning this stays between the client and counsellor. If the counsellor feels the client is at risk to themselves or somebody else, they may break confidentiality to seek further support. Your counsellor will let the client know if they are going to do this.

15 ) If additional contact details have been provided for a referrer (this might be a parent or professional that has supported with the referral) we will share information about attendance to sessions with this person. We may also contact them if we feel we need to break confidentiality, if we have reason to believe the client is at risk of harming themselves or anybody; this also includes any follow up relating to these incidences.

16 ) It’s really important that children/young people have a private and confidential space to share how they are feeling and build a relationship with their counsellor. For this reason, counselling sessions must be 1-2-1 with the client and counsellor. A parent/professional may only attend the assessment session with the young person if they wish to have support for this appointment.

17) As a parent/guardian/ organisation/young person completing a referral to Stop.Breathe.Think, you must confirm that  parental consent has been gained from BOTH parents/legal guardians (if applicable) .

18) By completing a referral to Stop.Breathe.Think, you agree to receiving text correspondence for appointment reminders and booking links.

19) If the young person who is seeking counselling with Stop.Breathe.Think is involved in any family court, or criminal proceedings you must let us know as soon as possible. Please note that it is your responsibility to inform any Key Workers that you’re accessing support with us.

20) Please note that Stop.Breathe.Think can not provide support with issues relating to adoption.

GDPR statement

Your personal data is important to Stop.Breathe.Think. We only store what we see as the minimum required information about you and your sessions. This is stored on our secure, private, GDPR compliant, clinical system called Iaptus. In addition to the information collected from your referral and intake, which includes your email, phone number, address, GP details and emergency contact, Stop.Breathe.Think keeps a very brief factual overview of what is bringing you to counselling from each of your sessions. These notes and your contact details are kept on Iaptus.

 We gather anonymous data and insights into the support we are providing to young people via Iaptus and the information you provide helps us to understand the impact the service is having. The data we generate is not identifiable to you. All clients are given a unique reference code to ensure an extra layer of security and anonymity.

Stop.Breathe.Think keeps client records for up to 7 years, after which they will be destroyed. Please know you have the right to request and check any information Stop.Breathe.Think keeps about you. You can do this by emailing admin@stopbreathethink.org.uk and we will respond within 7 working days.
